Symptom Tracker was designed to facilitate assessing patient outcomes in real-time providing clinicians with concise reports, summarizing progress, baseline, test scores, as well as adverse responses to treatment if desired.
How SymtpomTracker Works
Clinicians are provided with access cards which are distributed to clients as they begin a treatment program. Upon logging on, the system generates a unique account for each individual. A valid e-mail account is optional but if entered enables the system to send weekly reminders to patients to complete measures. Accordingly, no personal information is required.
The system is extremely flexible and can be configured to administer any number of questionnaires, in different languages, and at different time frames (e.g., weekly, daily, monthly, or after the fourth login), irrespective of the respondent (e.g., parent, child or professional).
Adverse side effects to medication (e.g., blurred vision or rashes) can be monitored in real-time and reported via e-mail alert to the clinic or via a pop-up alert to patient. Similarly, satisfaction and / or utilization surveys could be tied to the first or second visit, if that is desired. Or the system might be configured only to evaluate just the first visit.
Upon completion of the scheduled questionnaires, the system generates a 1-page report plotting symptom scores over time, The report can be directed to the clinic or can be printed immediately by the patient (to facilitate compliance and minimize clinic workloads).
